Therapeutic treatment that is highly specialized
It is the mission of Project Transition to help those in recovery from serious mental illness to reclaim independence and live meaningful, sustainable lives. Our residential treatment community model offers medication management, skill development, wellness coaching, relationship building and supervised independent living. We promote sustainable recovery through three main areas of focus:
- The treatment and self-management of psychiatric difficulties (with or without co-occurring substance use)
- The restoration of functioning, relationships, and self-direction
- Reintegration into the community
We believe that personal recovery can and does occur, even in the presence of painful struggles and setbacks. Here at Project Transition we practice the concept of a “human bridge” — as a person navigates their journey toward increased autonomy, his or her relationships with staff and peers remain constant and meaningful. Over time, as the intensity of support and the need for supervision gradually recedes, important relationships are sustained and confidence grows. When setbacks occur, multiple safety nets are already in place in the form of “community”.
