For over 28 years, Project Transition (PT) has provided help and hope to adults with psychiatric problems that have been recurrent and persistent. Many of the people we work with have felt failed by previous treatment efforts, including psychiatric hospitalizations, medications, and drug and alcohol rehabilitation programs. We have Therapeutic Communities located throughout southeastern Pennsylvania.

Approximately 200 highly accomplished and well-educated health care professionals work with Project Transition including psychiatrists, psychologists, social workers and therapists.
