Admissions and Referrals
- We work with young adults and adults who have a psychiatric diagnosis,
which may include a personality disorder and/or substance abuse.
- Our approach to determining the “fit” between Project Transition and an applicant is highly individualized. Confidentiality is assured.
- A phone call to Project Transition starts the ball rolling. We receive inquiries from individuals, family members, and professionals. As we answer questions about PT, it’s helpful for us to have a sense of the person – his or her strengths, needs, goals, diagnosis, and history.
- We’ll also discuss funding. Our services are paid for by family
members, selected insurance carriers, some Pennsylvania Medical Assistance
plans, and other sources.
- Another step of our evaluation is a review of recent clinical reports,
such as evaluations and treatment summaries. With consent, these documents
may be faxed to us (215-997-1550). We may also ask permission to follow-up
with a therapist, hospital social worker, psychiatrist, or others.
- It’s always a good idea for the person and family members to visit
us. This an opportunity to meet some of our staff and members,
and to see our apartments. If it’s is not feasible, we may visit
the person at home or at a treatment facility.
- Admission decisions reflect consideration of the individual’s suitability and readiness. Timing and motivation are important factors, as well.
- Project Transition is not “all things to all people”. If
our program is not a good fit now, or if there are funding limitations, we
will do our best to suggest alternative interventions.
