For over twenty-five years, Project Transition (PT) has provided help and hope to adults with psychiatric problems that have been recurrent and persistent. Many of the people we work with have felt failed by previous treatment efforts, including psychiatric hospitalizations, medications, and drug & alcohol rehabilitation programs.

The Members of Project Transition

Individuals referred to Project Transition have been through a lot. So have their family members. Multiple efforts to support the person have left everyone exhausted. The individual may be described by others as being challenging, difficult, or treatment resistant.

We work with people (ages 18+) who have a psychiatric diagnosis. Our programs reflect three areas of specialization:

Members of PT and their families come to understand that meaningful gains occur over the long haul. Lengths of stay vary – the average range is 8-16 months.

Overview

Project Transition cultivates trust and hope through relationships, treatment, skill development, wellness training, and supervised independent living. Our therapeutic communities are based within attractive suburban apartment complexes in southeastern Pennsylvania (Warrington, Bucks County and Chestnut Hill, Philadelphia). Roommates share a fully furnished apartment; they have access to a swimming pool, health club, shopping, and leisure attractions. Program activities and services, which are provided at the apartment complex, are augmented by our 24/7 crisis response team (PT counselors, program directors, and psychiatrists). Family activities include a welcoming orientation, ongoing progress meetings, multi-family support meetings, and quarterly educational seminars. These events are described in News and Events.

Our staff team consists of experienced professionals: Psychiatrists, Psychologists, Psychiatric Rehabilitation Counselors, Program Managers, Certified Addiction Counselors, and a Wellness Coordinator. Most of our clinicians have over twenty years of experience.
