Project Transition Job Description:
Psychiatric Rehabilitation Counselor
This person has a pivotal role at Project Transition (PT). Each Counselor is affiliated with a designated PT community. The core duties involve extensive day-to-day contact with PT members. Most Counselors have primary responsibility for 5-7 persons, yet they interact with (and actively support) the entire PT community. The responsibilities, described below, occur through relationships that are consistently informed and validating – a cornerstone of PT.
Diverse challenges and responsibilities are the norm for our Counselors. The three primary job functions are:
- Active participation in program activities and the supervision of PT members.
- Developing, implementing, and documenting activities related to treatment, skill development and coaching, psychosocial fitness, and the therapeutic community.
- Participation in our crisis response system.
Specific responsibilities:
- Welcoming and orientation of new members of PT.
- Individual assessment, skill & motivational coaching, and counseling.
- Active participation in (or facilitation of) workshops and other group modalities.
- Consultation with external agency representatives, family members, program psychiatrist, and psychologists.
- Documentation of monthly and weekly progress reviews, as well as those relevant to funding authorization.
- Implement medication administration and documentation procedures.
- Crisis response: with appropriate training, cover crisis pager as assigned and respond to emergent calls.
- Attend at least 30 hours of continuing education annually.
- Other duties as needed.
Qualifications:
Required: minimum age 21 and a Bachelor’s degree.
Preferred: Bachelor’s degree and three years of relevant experience OR Master’s
degree and one year of relevant experience.
