Start the Admissions Process

Contact our Admissions Department via email 24/7 or call our office at (215) 997-9959, Monday - Friday, between the hours of 8 a.m. - 5 p.m.

If You Are Currently Hospitalized

  • Immediately provide Project Transition's contact information to your case worker and request they speak to our Admissions Team to learn how to make a referral. 

If the referral is approved before you are discharged, you may be assessed by our clinical team for readiness to enter the Project Transition Program at the hospital, or if permitted, you may come to do a tour and assessments at the recovery center.

If You Are Currently in the Community (not hospitalized)

If you are not hospitalized and currently living at home, with supports, in a shelter, or homeless you can do the following:

  • If you are a CBH Member: download a copy of the Project Transition/CBH referral form and provide it to your social worker, case manager (or ICM), outpatient provider (ie: psychiatrist). Ask your provider to contact Project Transition and speak to our Admissions Team to learn next steps.

  • If you are a CCBH-Berks Member: download a copy of the Project Transition/CCBH referral form and provide it to your social worker, case manager (or ICM), outpatient provider (ie: psychiatrist). Ask your provider to contact Project Transition and speak to our Admissions Team to learn next steps.

  • If you are a BlueCare or United Member: have your social worker, case manager (or ICM), or outpatient provider (ie: psychiatrist) to contact Project Transition Admissions or you may contact us directly to schedule assessments and a tour. 

  • If you are interested in paying privately, contact Admissions for a copy of the Member Financial Agreement. If paying out of pocket, a referral submission to the health insurance provider is not needed.

Regardless of insurance, you must complete a pre-admission packet (download the packet for Pennsylvania or Tennessee) and return it to Project Transition.

Members in Pennsylvania (with an approved referral) visit the program for a tour and pre-admission assessments. Tennessee Members do not require a referral from the insurance company to tour the recovery center and be assessed for readiness.

Your First Contact With Us

Admissions Department to ensure the services we provide are in line with the needs you have, and the challenges you are facing. Our Admissions team will describe our program settinng, living arrangements, expectations and commitment of the daily program, and average length of participation. Everything we do is member-driven and person-centered in nature. It is the Admissions Department's goal to determine a fit between the services we provide and how they align with your needs and goals.

Complete the Pre-admission Packet

You'll complete a pre-admission packet (download the packet for Pennsylvania or Tennessee), which you'll return to us. The consents must be completed for every hospitalization or behavioral health provider you have history with, your parole or probation officer, and any supports in your life you want us to be able to speak to. You must also have a physical completed within the last six months.

Schedule Tour and Pre-admission Assessments

If able, you will visit the Project Transition Recovery Center where you will participate in your therapeutic and clinical treatment, as well as tour the apartment complex you will reside in. Your time with our staff will include:

  • Meeting your admissions counselor and completing any incomplete or missing paperwork in the pre-admisison packet
  • Complete a drug and alcohol screen
  • A clinical evaluation, safety plan, suicide risk assessment, and drug and alcohol assessment with a therapist
  • Treatment Plan Development
  • Milestones Assessment
  • Apartment tour and self-preservation test

Time at the recovery center will take approximately four hours from start to finish. Assessments for readiness for the program at Project Transition are typically available within 72 hours of your visit.
